At  AHC&SPC, your medical records are yours to review and send to other medical providers. Use the information and links below to find out how to release your medical records.

Remember, our secure patient portal is available for you to conveniently review and/or print your medical records from the comfort of your own home! 

 

To have a copy of your medical records sent to another medical facility or to obtain a copy for personal reasons, please complete the following steps:

  1. Download and complete and sign the AHC&SPC Authorization to Release Medical Records Form. (Click the button above)

PLEASE NOTE
If you are requesting a copy of your medical records in person or by mail/fax as described above, there may be a fee associated and charged. If you are only requesting a copy of your last office visit note, AHC&SPC charges no fee. However, for more multiple visit notes or your entire AHC&SPC medical record, we charge a $30 fee. This fee is in accordance with the applicable state and federal regulations as described in the websites below:

Record Request FAQ

It generally takes 7-10 business days from the receipt of the request to complete a request for records.
If you are only requesting the records pertaining to your last visit at AHCSPC, there is no fee. If you are requesting additional records, we charge a $30 flat fee, which results from rules governed by the state and federal government. Please see the links above for more information.
Yes, for AHCSPC, outside records are re-released for all record request types, unless specifically instructed otherwise by the patient/requester.
No, your AHCSPC medical records are grouped and stored in the same Electronic Health Records system, which is central and common to all AHCSPC facilities.
The authorization form expires 90 days from the date it is signed, unless the requestor indicates a specific date on the form.
