Athens Heart Center offers program that engages patients in their own health care

Patient engagement isn't just a buzz word at Athens Heart Center and Specialty Clinics. Through a patient-centered program to monitor health information, AHC is putting patients in the driver's seat on their road to health.

Through its Remote Patient Monitoring (RPM) program, Health Wealth Safe, AHC invites its patients to take a highly active role in their health care, a development in patient engagement, which can lead to better health outcomes and reduced costs over time.

According to a 2013 health policy brief by Health Affairs, a peer-reviewed journal on health policy issues and research, evidence shows that patients who are more actively involved in their health care - like patients in the Health Wealth Safe RPM program at Athens Heart Center - show better health care results and incur lower costs.

Health Wealth Safe's RPM program allows patients to record and track important health care data over time from the comfort of their own home, making health care more personalized. This data may include blood pressure, heart rate, pain levels, and other information crucial to monitoring a patient's progress. This data is then used by physicians and clinical staff to determine a patient's health care needs, greatly increasing the quality of care. RPM promotes preventive care, as health concerns and issues are identified early, which can lead to a reduction in health care costs, as well as improved patient experiences.

RPM also empowers patients to manage their own health and to make important health care decisions. Through RPM, patients are provided a user-friendly platform for tracking their health data, information that is vital to their care.

On Jan. 1, 2019, the Centers for Medicare & Medicaid Services (CMS), which oversees Medicare and Medicaid, began reimbursing physicians for additional remote physiological monitoring protocols. In a fact sheet, the CMS cited better accessibility, quality, affordability, empowerment, and innovation for this and other changes to its physician fee schedule for 2019.
