Physicians Circle For Patients (Patients Assistance Program)
Athens Heart Center and Specialty Clinics is committed to providing care for all our patients. We created the Physicians Circle For Patients, a patients assistance program, to help low income patients with costs like copays, prescriptions, etc. Please see the flyer below for more information.
How to Get Records of a Patient You are Treating:
Please contact our office via the phone numbers provided at the bottom of the page. We will work with you quickly to send patient records to your office, via fax or secure email.
HIPAA Law (Click Here) allows you to discuss patient medical information and share patient medical records among the physicians who are in the patient's care team. If our record shows that patient has been referred by you, or the patient has told us that you are in his/her care team then we will share his record with you. We will also help in managing his/her medical care.
If our record does not show that you are in the patient's care team, than we may need a consent form from the patient to release patient medical information to you. Form. Once we have received the signed consent form indicating the patient wants us to release their medical records to you, then the medical record can be faxed to you immediately.
